Final TAA Evaluation Survey

All TAA participants are asked to complete the TAA Final Evaluation Survey by July 31, 2021. Final credit for attendance at the Tennessee Arts Academy will be awarded once this form is received. Participants will receive either a certificate verifying a minimum of 36 hours was spent in Academy sessions or if the 36-hour minimum was not reached, a letter of attendance verifying the exact number of hours spent in Academy sessions.

The Final TAA Evaluation Survey will be made available on this site starting at 1 PM on July 23, 2021. The survey must be submitted by the end of the day on July 31, 2021.

Beginning on July 23, 2021 at 1 PM, a link will be available to access the Final TAA Evaluation Survey.
